Estou usando o Btrfs 3.12 e cada vez que eu excluo um instantâneo, todo o volume btrfs se torna "Somente leitura". Então eu desisto e tento um
btrfsck --repair

Você encontrou dois problemas, que não precisam estar relacionados entre si.
dmesg não dá nenhuma pista sobre isso.
btrfsck --repair falha com o sinal 6 (conforme mencionado no arquivo .crash) Isso é um bug.
