Quando eu uso o FFMPEG para converter um vídeo do MP4 para o M3U8, usando o seguinte comando,
ffmpeg -i op.mp4 -b:v 128k -flags -global_header -map 0:0 -map 0:1 -f segment -segment_time 4 -segment_list_size 0 -segment_list op.m3u8 -segment_format mpegts op%05d.ts
Eu recebo o aviso abaixo:

O M3U8 é gerado, mas somente o áudio está chegando no quicktime player.

Então, eu gostaria de saber

Motivo por trás desse erro. É um mp4 corrupto ou problema durante a conversão. O m3u8 é jogável não é jogável no quicktime corretamente. Apenas o áudio aparece, sem vídeo.
