Alguns dias atrás, meu servidor linux apache ficou sem memória. O servidor é um convidado xen. O servidor matou todos os meus processos, exceto ssh (até mesmo cron, monit, syslog).
Um minuto antes de ficar sem memória, um script meu salvou a saída de vários comandos. Aqui estava a saída:

Além disso, como era uma da manhã, quase não havia conexões do apache na época.
Como você pode ver, o swap não estava sendo usado. Ainda havia um pouco de ram livre (de acordo com o monit).
Alguma idéia de como posso descobrir a causa?
