Há apenas alguns poucos RAM sobrando e ainda usando SWAP muito baixo até que ele falhe e a RAM seja liberada e eu tenha outra falha em algumas horas:

O que é / proc // oom_adj?

Como a demanda de memória muda - um aumento repentino pode indicar que você está atingindo o limite de 2 Gb. Um aumento lento implica que você definiu maxclients muito altos e provavelmente precisa de mais hardware. Não faz sentido usar ferro grande como servidor da Web - use várias caixas menores.
