Depois do ubuntu install + software raid 0 config, vemos: rejeitando I / O para dispositivo offline

Question

Estou tentando descobrir o que esse erro rejecting I/O to offline device é, como visto no final deste syslog.

Idéias:

• Problema com a configuração do RAID-0 (temos dois discos de 4 TB no RAID-0 e vejo um aviso sobre isso)
• CD virtual - não há CD neste servidor, mas eu tinha um CD virtual conectado a uma ferramenta IPMI / KVM para a instalação. Isso não deve ser anexado agora, mas vejo algumas mensagens sobre isso.

Além das notas acima, esta é uma nova instalação do Ubuntu 12.04 que foi feita através do netboot e através do console IPMI / KVM.

Atualização:

@jordanm & outros - correto, eu apenas confirmei que este é o dispositivo de CDROM, mas este é um "cd virtual" que foi criado por uma sessão KVM anterior via IPMI no servidor. Não existe mais, alguma ideia de como posso me livrar disso? Ou não tentar acessá-lo, não sei por que ele está lá.
