É uma sintaxe não óbvia, pois parece um comentário, mas os sudoers permitem incluir regras em outro diretório com a diretiva:
#includedir /usr/local/etc/sudoers.d
É provável que sua regra nopasswd exista em um arquivo nesse diretório.

Acho que o sudo normalmente solicita a senha desse usuário atual, desde que o usuário seja membro do grupo wheel.
