Eu tenho usado proxies do squid com autenticação em servidores VPS rodando no Ubuntu 12.04, mas por alguma razão nunca consegui fazer isso funcionar no 14.04 usando o mesmo arquivo conf, e eu não consigo descobrir o que estou fazendo errado, mas estou supondo que é algo no arquivo conf que precisa ser alterado.
Meu arquivo conf atualmente se parece com isso e funciona bem em 12.04 (por favor, não removi "basic" da parte ncsa_auth na linha 2, mesmo que alguns guias digam o contrário):
