Eu tenho uma unidade flash USB de 128GB aqui que não funciona mais. Quando eu o insiro, demora uma eternidade para montar, e para sempre significa que ele não vai montar no final.
Usando o Terminal, recebo a seguinte mensagem de erro:
$ diskutil eraseDisk JHFS+ TEST123 /dev/disk4

E isso:

Esse bastão foi usado em um receptor Yamaha com conector USB para reproduzir MP3s. Ele ainda vê as pastas, mas quando eu tento abrir pastas com arquivos de música, ele dá um "erro de acesso".
Tudo que eu quero é formatar o disco. O que há nisso não importa. Como posso fazer isso funcionar?

O reparo do Diskutil deu o seguinte resultado:
