Log de falha de asserção do MySQL 5.5 (Percona) .. o que causaria isso? [ATUALIZADA]

256 GB de RAM, 64 Core, AMD executando o Ubuntu 12.04 com o Percona MySQL 5.5.28.

Abaixo está a falha de declaração. Acabamos de ter uma segunda falha de asserção (diferente "no arquivo", posição, etc) durante a execução de um grande conjunto de inserções.

Após a primeira falha, o MySQL reiniciou após uma reinicialização apenas - depois de repetir continuamente o mesmo erro após tentar se recuperar.

Eu decidi fazer um mysqlcheck com -o para otimizar. Uma vez que estas são todas as tabelas Innodb (tabelas muito grandes, 60 + GB) isso faria uma tabela de alteração em todas as tabelas.

No meio disso, a falha de declaração abaixo aconteceu novamente:

Atualmente estou executando um despejo para ver se o problema ressurge. - mysqldump completou bem. Agora estou tentando restaurar o banco de dados inteiro do despejo.

EDITAR: o mysqldump foi concluído sem problemas. Por que essas falhas ocorrem em uma tabela alternativa?

Eu tive uma falha semelhante na mesma linha de código, com poucas explicações sobre o motivo.
