Ubuntu + Luks: Grupo de Volume 'vgcrypt' não encontrado (drop to busybox)

Problema: Eu segui as instruções para instalar o Ubuntu 16.04 em um sistema EFI com LUKS + LVM deste upvoted question on AskUbuntu , mas acabei com um sistema inutilizável.

A única coisa que eu mudei foi o tamanho / ordem das partições e o dispositivo no qual realizei o particionamento.

Esta é a saída de lsblk , o disco de interesse é /dev/sdb/ :

O lvm na partição luks /dev/sdb3 é chamado de crypt e contém swap , raiz e casa .

Os discos restantes são usados pelo windows 10 , então eu os deixei intocados.

Depois de obter o uuid de /dev/sdb3 de blkid :

Eu atualizei o arquivo /etc/crypttab dentro do novo sistema, conforme recomendado pelas instruções:

Como desejo que a senha seja solicitada toda vez, não configurei um arquivo de senha.

Atualmente, meu /etc/fstab/ é o seguinte

AFAIK, este arquivo foi gerado automaticamente durante o processo de instalação e parece bem.

Erro:

Durante o processo de inicialização, eu sou deixado no busybox com a seguinte mensagem de erro:

Volume group not found
Cannot process volume group vgcrypt

Estou um pouco confuso com este erro, até agora googling não parece retornar nenhuma resposta útil entre os resultados da primeira página.

Pergunta: alguém saberia o que há de errado na minha configuração e como posso corrigi-la?

Eu já tentei usar

# update-initramfs -u -k all 

E verifiquei que lvm2 está instalado no novo sistema, que é o Ubuntu 16.04 com o kernel 4.4.0-31-generic .
