volume de LVM corrompido, não pode ser montado após a reinicialização

Eu criei um volume LVM usando este guia Eu tenho 2x2TB HDDs para um total de 4TB (ou 3.64TB utilizável). Eu criei o volume e reiniciei. Tudo estava funcionando bem. Em seguida, copiei 1,6 TB de dados no volume e, depois de reiniciar, o volume não pode ser montado. Aqui está a saída durante a inicialização:

Isso aconteceu comigo três vezes e não consigo encontrar o problema. Eu já tinha tentado um fsck -y da última vez, mas ele continuou consertando e consertando blocos, e no final os dados foram perdidos.

Os HDDs funcionaram bem entre os dados copiados e a reinicialização. As velocidades de leitura / gravação eram boas, e todos os arquivos que abri funcionavam sem problemas. Eu não acho que seja um problema de hardware. Existe algo que eu possa fazer para restaurar o volume LVM?
Eu deveria especificar que estou executando o Debian 7.

Seu / dev / sdb é claramente torrado (tem toneladas de setores defeituosos realocados e pendentes). Você precisa substituí-lo.
