você tem smtpd_recipient_restrictions duas vezes, e a última é a que está sendo usada e não tem permissão_sasl_authenticated
Eu tenho em /etc/postfix/main.cf:
smtpd_sasl_auth_enable = yes
smtpd_recipient_restrictions = permit_sasl_authenticated, permit_mynetworks, reject_unauth_destination
e o SASL parece estar funcionando:
$ testsaslauthd -u yang -p ... -f /var/spool/postfix/var/run/saslauthd/mux -s smtpd
0: OK "Success."
mas estou recebendo o "Acesso de retransmissão negado" ao se conectar de fora da rede (e, por exemplo, o localhost ainda funciona bem):
